The folded inner sheet seems to have been missing already from the start on several of the UK pressings. I remember when I bought mine copy when the album was released and it did not include the sheet. I thought it was the Swedish record store who had made the mistake but after that I have seen a lot of UK pressings without the sheet. Of course some might have disappeared over the years but too many seems to be missing. I have not seen any special promo pressings of the UK lp, or with promo stickers, but there are standard copies that were sent out for promotion with promo material - photo, review from Kerrang and 4-page biography.
